Two concentric rings of small round diamonds surround the oval sapphire, separated by a raised white gold rim so the halos read as distinct circles rather than one wide band of pave. That double halo is what pushes the head out to cocktail width: the center stone is large to begin with, and the two frames carry the outline well past it on every side.
The sapphire weighs 4.30 carats and the diamonds 0.48 carats between them, which is a high accent share for a stone of this size and explains how bright the surround reads against so dark a blue. White gold runs throughout, including the broad shoulders that step down from the disc into the shank at size 7 US. On the hand the head covers most of the width of the finger.
